If your home has a single-glass-paned Superior or Lennox brand gas fireplace installed between Feb. 6, 2004 and Jan. 11, 2011, you may be eligible to receive a free protective screen for the front of your fireplace. Single glass panes have been associated with serious skin burns.

If you plan to make a claim under the class action settlement, you must submit your claim form prior to Aug. 1, 2011. To file a claim online, click here. You must submit one claim for each fireplace and provide the brand, model number and serial number for each. This information is usually found behind a door at the bottom of the fireplace. Claim forms may also be downloaded and mailed. Since California claimants have a slightly different settlement, you may ignore that question on the form.
